A Note from the Nurse:

The following are health requirements for next school year -

Kindergarten:  Physical, Immunizations, Dental Exam & Eye Exam
Second Grade:  Dental Exam
Sixth Grade:  Physical & Dental Exam
Ninth Grade:  Physical & Immunizations

All Students participating in sports or tryouts must have a current physical on file.  The sixth and ninth grade school physical can be used as a sports physical.  Please keep in mind that a sports physical is valid for one calendar year and must cover the entire sports season your child participates in.
